LIABILITY WAIVER AGREEMENT - Virtual Journeys

I understand that yoga & flow (poi, hoop) movement require and include physical movements. I realize that these activities also provide relaxation, stress relief, stress education and awareness. As with any physical activity, the risk of injury, even serious or disabling, is always present and cannot be entirely eliminated. If I experience any pain or discomfort, I will notify the SOUL FLOW ALCHEMY team, adjust my posture, stop the movement, and listen to my body. I will not push my body too far and will ask for support from the teacher if needed.

I understand life coaching requires mental & emotional involvement. I realize that participating in a life coaching program can provide relaxation, stress relief, stress education, awareness, and truths never before realized. As with any life coaching program, the possibility of past emotional trauma appearing during the journey, is always present and cannot be entirely eliminated. If I experience any pain or discomfort, I will notify the SOUL FLOW ALCHEMY team and listen to my body. I will not push my emotional body too far and will ask for support from the teacher if needed.

Yoga, the Flow Arts, and the information shared in this virtual journey aren’t substitutes for medical care or diagnosis, but can work well in conjunction with traditional medical care. I will practice this only after discussing it with my doctor and gaining their approval. I affirm that I alone am responsible to decide whether to practice whatever I learn in this multiple week virtual class. I hereby agree to irrevocably release and waive any claims that I have now or hereafter may have against SOUL FLOW ALCHEMY, JINJU DASALLA, NOVA DASALLA, or ANY OF THEIR REPRESENTATIVES. 

I, hereby agree to the following:

1. That I am participating in this multiple week program offered by SOUL FLOW ALCHEMY, where I will receive information and instruction about these practices. I understand that they require physical exertion, or mental and emotional energy, which may be strenuous or intense and may cause physical injury or possibly bring up mental or emotional trauma. I am fully aware of the risks involved.

2. I understand that it is my responsibility to consult with a physician or mental health professional prior to and in reference to my participation in this program. I warrant and represent that I am physically and emotionally fit and have no medical condition that would prevent my full participation in the program. I understand that this program is no replacement for therapy or is any way a cure for anything.

3. In consideration of participating in this program, I agree to assume full responsibility for any risks, injury or damages, which I might incur as a result in my participation.

4. I knowingly and voluntarily waive any claim I might have against SOUL FLOW ALCHEMY, JINJU DASALLA, NOVA DASALLA, or ANY OF THEIR REPRESENTATIVES, for injury or damages that I may sustain as a result of participating in this program and understand that SOUL FLOW ALCHEMY, JINJU DASALLA, NOVA DASALLA, or ANY OF THEIR REPRESENTATIVES make no warranties or claims of any of the information shared in the program.

5. I, my heirs or legal representative forever release, waive, discharge and covenant not to sue SOUL FLOW ALCHEMY, JINJU DASALLA, NOVA DASALLA, or ANY OF THEIR REPRESENTATIVES, for any injury or death caused by their negligence or other acts.

I have read the above release and waiver of liability and I fully understand its contents. I fully and voluntarily agree to the above terms and conditions.
